Write a handoff document summarising the current conversation so a fresh agent can continue the work. Save to the temporary directory of the user's OS - not the current workspace. Include a "suggested skills" section in the document, which suggests skills that the agent should invoke. Do not duplicate content already captured in other artifacts (PRDs, plans, ADRs, issues, commits, diffs). Reference them by path or URL instead. Redact any sensitive information, such as API keys, passwords, or personally identifiable information. If the user passed arguments, treat them as a description of what the next session will focus on and tailor the doc accordingly. ## Post-incident capture gate Before writing the handoff, check if the session encountered any failures: - **P0/P1 failures** (production incident, data loss, security failure, broken CI gate): if a committed root-cause artifact (ADR or incident-log entry) does not yet exist, flag it as an open action in the handoff: "OPEN: incident capture required before next task — root cause, fix applied, preventive action." - **P2/P3 failures** (CI flake, test regression): write a brief memory note (if not already done) and include it in the handoff context. - **Repeat root cause** (same failure ≥2× in 14 days): flag as requiring an ADR + prevention rule, not just a memory note.
